Registered Nurse Role $2,130 per year About the Role We are
looking for a compassionate and experienced Registered Nurse (RN)
to join our Program of All-Inclusive Care for the Elderly (PACE)
team. The RN will provide direct nursing care, coordinate services,
and support participants in maintaining their independence,
dignity, and quality of life. Responsibilities - Conduct initial
and ongoing nursing assessments for PACE participants. - Develop,
implement, and evaluate individualized care plans in collaboration
with the interdisciplinary team. - Provide skilled nursing care,
including wound care, medication administration, and chronic
disease management. - Coordinate care across multiple settings
(clinic, home, and day center). - Educate participants and
caregivers on health conditions, treatments, and self-care
strategies. - Serve as a resource and advocate for participants,
ensuring their needs are met. - Maintain accurate documentation in
compliance with federal, state, and PACE regulations. - Participate
in interdisciplinary team meetings to promote holistic,
