Job Description:
AccentCare is looking for a dedicated and compassionate
Registered Nurse (RN) to join our team as a Patient Care Manager in
the Home Health division. This role offers a $15,000 sign-on bonus
and is pivotal in ensuring superior patient care within our
community. The Patient Care Manager will oversee a team of nursing
and allied health professionals, ensuring the delivery of
high-quality home health services, managing patient care plans, and
working closely with families and caregivers. Lead, supervise, and
evaluate the home health care team to ensure compliance with
regulatory requirements. Develop, implement, and review care plans
in collaboration with patients and their families. Coordinate with
healthcare providers to facilitate optimal care and address patient
needs. Manage caseload, documentation, and care coordination
efficiently. Ensure continuous improvement in the quality of
services provided. Maintain a safe environment in accordance with
health and safety standards. Strong leadership and team management
skills Excellent communication and interpersonal skills Proficient
in using healthcare information systems Ability to handle stressful
situations professionally Knowledge of home healthcare regulations
and policies Medical Insurance Dental Insurance Vision Insurance
401k Valid RN license in the state of practice CPR certified
Preferably certified in Home Health or Hospice (CHPN or CHHCM)
